Make SmoothStreaming manifest more robust. · ddd2d004

- Skips unrecognized elements rather than crashing.
- FourCC treated as required for video and optional elsewhere,
  as per the SmoothStreaming spec.
- Only parse initData text when we're actually in the ProtectionHeader element
